Primary Care Wirral have expanded their Wellbeing Practitioner Awareness Campaign with new videos to celebrate Link Worker Day 2020.
We are supporting Link Worker Day 2020 by sharing a wide range of short Q&As clips aimed at offering an insight into the day-to-day work of our vital Wellbeing Practitioners (AKA Social Prescribing Link Workers).
Supporting Practices and PCNs
Wellbeing Practitioners play a crucial role in relieving pressure from practice staff by providing social support for patients in need of non-medical treatment.
1 in 4 GP visits are related to social issues including concerns over emotional problems such as loneliness and depression and practical concerns including money worries and housing support.
The hard work of Wellbeing Practitioners provide a vital network of help for patients needing specialised assistance.
Support is delivered in a calm and person-centric manner, with the of facilitating users access the help they need as quickly as possible.
Teamwork is an essential asset to any successful NHS organisation and our team of Wellbeing Practitioners are right at the heart of PCW’s efforts in supporting GP practices and PCNs to provide holistic patient healthcare across Wirral.
