Who should attend?
This free* Chronic Disease Management Update course is for health care support workers who would like to stay up to date with the latest developments in chronic disease management.
The training is specially recommended for Health Care Assistants (HCAs) who already have an understanding of chronic disease and the conditions which will be covered during the course (see ‘Course Content’ section below).
Please, be aware that as the course will focus on recent changes and trends, a certain level of knowledge and/or experience dealing with patients with long-term conditions will be assumed.

What will I learn?
This interactive workshop by Louise Wainwright, is a great opportunity to stay up to date with the latest developments in care provision for patients with long-term conditions.
You will gain a deeper understanding of how chronic disease management affects clinical practice, with an emphasis on its impact on HCAs’ day to day operations.
The course will also cover relevant guidelines to guarantee excellent care delivery for various permanent health conditions, including diabetes, kidney disease and heart disease.
With over 15 million people in the UK living with chronic illnesses and the number set to rise, this training session ultimately aims to help you feel more confident in your future interactions with these patients.
Course Content
A mix of lecture, group work using case studies, Q&A and directed learning will be used to explore the impact of chronic disease management on clinical practice and day to day operations for the following conditions:
- AF and Anti-Coagulation
- Heart Failure
- Hypertension
- Liver Disease
- Diabetes
- Chronic Kidney Disease
- Cardiovascular Disease
Assessment
There is no assessment included as part of this training course.
However, it is the responsibility of the delegating health care providers to ensure that competences are achieved and maintained using whatever practice protocol is in place (i.e. appraisal).
