Healthier South Wirral (HSW) Primary Care Network (PCN) has received the Excellence in Partnership Wirral Community Health and Care NHS Foundation Trust (WCHC) Heart Award.
The winners were nominated by Dave Hammond from WCHC and Caroline Jones from CWP, and competed in the Excellence in partnership category alongside Katy Fisher from Learning and Organisational Development and the Dental Special Care Theatre General Anaesthetic Team.
Trailblazers in Partnership Working
The award is a recognition for an individual or team who has demonstrated excellent partnership working internally or externally, for example developing equal relationships with other partner organisations or improving outcomes for staff, patients and service users.
Hammond highlighted HSW’s promotion of a forward-thinking model of patient care based on the “establishment of excellent relationships, good information sharing and care coordination across a variety of system partners, in particular General Practice and Community Teams”.
Jones described HSW as “the true essence of excellence in partnership” and praised the team’s collaborative approach to working, including regular network meetings, joint education programmes, patient engagement events and process mapping.
The award was delivered by WCHC Chief Executive, Karen Howell, who travelled to The Orchard Surgery to personally congratulate the South Wirral’s GP network team.
“You are an example to the Wirral system, you really are”, said Howell.
“The partnership working that you have undertaken and exemplified for the rest of Wirral is a real demonstration of excellence.
“Please, keep setting the standards, you are doing an amazing job and we are very, very proud of working with you in Primary Care, but also on behalf of these guys out here, the people of Wirral”.
The award was received by HSW Clinical Director Dr Nazia Syed-Mohammad, who stressed HSW “is a perfect example of a team effort and the entire credit goes to the entire team working together.
“It is a really collective sort of recognition for what we have done together, I am really amazed ”.
First Virtual Heart Awards
Originally planned for March 2020, the WCHC Heart Awards had to be postponed until July due to the increasing pressure of Covid-19.
In an effort to guarantee attendants safety and facilitate adherence to social distancing rules, the new celebration has come back in and innovative virtual format.
“We certainly did not want to lose the opportunity to celebrate and recognise our colleagues who were shortlisted for a Heart Award before this all (Covid-19 outbreak) began”, said WCHC Chairman, Professor Michael Brown.
“After all, this year’s event had promised to be better and bigger than ever before, with over 100 nominations across the eight categories.
“Although the March event had to be pulled because of Covid-19, I am thrilled we have been able to organise an alternative way for you to celebrate and recognise each other while staying safe.
“Our virtual awards, will allow us to share your stories, recognise your efforts and exceptional contributions you have made to the work of the trust”.
The ceremonies hosted by BBC North West presenter Roger Johnson, consisted of live events which took place across different dates and were broadcaster life on digital platforms.
